Patient Centered Medical Home Program
Fair Hill Community Physicians primary care practices participate in the state-sponsored Patient Centered Medical Home (PCMH) program. As a PCMH, we are focused on quality improvement and ensuring that patients have easy access to care. We also have a team of nurses who support high-risk patients in managing multiple chronic conditions and complex health needs and Community Health Workers who address social needs, such as transportation or housing.
